Using the Skilled Nursing Unit (SNU) Program Administration screen, you may add patients to the program, manage their treatment plan, and discharge them from the program. To access this screen, click the Programs option from the Global Menu, then select Skilled Nursing Unit.
- At the top of the Skilled Nursing Unit screen, you will find four tabs representing four queues:
- Admission
- SNU (Skilled Nursing Unit)
- LTC (Long Term Care)
- Discharge
Admission
- Patients are introduced to this queue through the Admission tab.
- To search for a patient, select the housing unit from the Housing filter. This filter defaults to All.
- To add a patient, select Add Patient in the upper right corner of the screen.
- Select a patient to work with by using the toggles to the left of the patient name.
- Once a patient is selected, an additional action dropdown will show just under the housing filter. From here, you will use the dropdown and select the action you wish to perform. Options include:
- SNU/LTC Provider Admission
- SNU/LTC Nursing Initial Assessment
- Move to Queue
- Note: Both the Provider Admission and Nursing Initial Assessment should be completed before moving the patient to either SNU or LTC.
- To complete the admission or assessment, select the appropriate action and then select Perform Action.
- When your document opens, select the appropriate group for which the patient will be assigned, SNU or LTC. Once that is selected, complete the form and select Save & Complete.
- As actions/forms are completed, a confirmation will show in the respective column. Once both the Provider Admission and the Nursing Initial Assessment are complete, the patient may be moved to the appropriate tab, SNU or LTC.
- To move a patient to one of the other tabs:
- Move the toggle
- Select Move to Queue
- Select Perform Action
- Select the desired Queue from the queue list
- Select Save
- Once a patient is moved to another tab, managing the patient remains the same process. Patient toggle is activated. Forms are chosen from the Action dropdown and Perform Action should be selected.
- In the SNU and LTC tabs, once a form is completed, select Refresh in the upper right corner and a timer begins indicating when the next assessment/form should be completed.
- SNU SOAP Notes should be completed every 72 hours
- SNU Progress Notes should be completed every 13 hours
- LTC Notes should be completed every 7 days
Discharge
- To discharge from SNU or LTC:
- Move the toggle
- Select Move to Queue
- Select Perform Action
- Select Discharge from the queue list
- Select Save
- Complete the SNU/LTC Provider Discharge form and SNU/LTC Nursing Discharge form.
- Complete the process:
- Move the toggle
- Select Remove from Dashboard
- Select Perform Action
